Login form using HTML5 and CSS3

This is an example how to create a simple login form using HTML5 and CSS3. This form uses pseudo elements (:after and :before) to create the multi page effect. These elements are rotated using the CSS3 transform property. This form uses HTML5 to make validation and submission easy.

Awesome! I only have couple problems with this: http://oi47.tinypic.com/ekmi4k.jpg
1. Both of username and password text-boxes icons are username's icon
2. Both text boxes icons are top of box, not center of it.

Those are only little problems, but that is for me really irritating.

So, i give for this 4,5/5!

PS. If you see that "download source file"-button is not "in" container, that was my mistake, i fixed that and same problem comes (1&2). Please answer my e-mail address: finnewyork@gmail.com.
